A profile manager is either the person who created the profile, someone who was added as manager at some point after it was created, or sometimes a person who adopted the profile after the creator stopped managing it. Basically, profile managers get notifications of changes to profiles they manage (which are, by default, on their watchlist) and they have more rights over the profiles they manage than other users do - profile managers can change a last name at birth, for example, whilst others can change current last name but not the LNAB.
