• There has been a question of whether her middle name was Diana, as one researcher has suggested, or a different name starting with a "T" (possibly her mother's maiden name, Thomas?). Among available documents, her name is always written "Helen T. Ells." The middle name is never included in its entirety. Shedding some light on this, a review of the original wedding register from Massachusetts clearly shows a "T." While this could have been an instance of the clerk being told "D" but hearing "T," the wedding was actually written up in a volume on the Unitarian Church (see citation below), in which her name was also spelled with a "T."

• Her exact birth date remains in question, though the year is known to be either 1870 or 71. Note, on her wedding day, 5 Jun 1893, she was listed as being 23 years old. Had she been born at any point in 1871, she could not possibly have been 23 in June of 1893. Thus her birth year had to be 1870. Hopefully a registry of her birth will turn up to settle the issue completely.
