Although the form indicates that his permanent address is Box 650 Hudson St., Manchester, Connecticut that is scratched off and a notation is made on the 2nd page that says "Change to Greenbrier, Tenn." However, the correcting entry is also scratched off and (on another line) the notation is made: "Ottawa, Kansas c/o F.H. Steward & Co." Although it's not clear what exactly was going on... his parents were living Ottawa Kansas and he did eventually go to Greenbrier, Tennesee - so the various place notations make general sense. 3 George gives his birth date as June 15, 1891 (although other records indicate it wa June 25, 1891) and says he was born in Ellenboro, West Virginia. He is working as an Office manager for C.K. Burr & Co. in Manchester.
