I hereby certify that I attended the deceased from Jan 10, 1935, to Mar 3, 1935, I saw her alive on March 2, 1935, death occurred at 7:45 am on March 3, 1935, in North LDS Hospital Salt Lake, Utah. She had an operation on January 18, 1935, for ceral spinal syscholsis - not known. An autopsy was performed by an MD in Salt Lake City with a finding of Amylopsin Lateral Sclerosis 1 month as the cause of death and the file number is 431255.
