Ralph Milbury McNeill was born 28 July 1881 to his father, Albert Clement MacNeill, and mother Cecelia Sophia Milbury, in Barton, Digby, Nova Scotia, Canada. His father was 36 at the time, his mother 31. Ralph was their second child, they having had a son, Albert Fletcher MacNeill, who was born 30 November 1876.
Ralph married Grace Coombs, daughter of Eseck and Jennie, in 1909.

At the time of his World War I draft registration in 1918, he was living in Newton Centre and as working as a chauffeur for a private family.[2]
