Sam is variously listed as Major, Colonel and Lt. Colonel. I've gone with Lt. Colonel, as per the research of Harry Duff.
He is also listed as being buried both at St. John the Baptist Anglican and Dixie Union Cemetery in Find-A-Grave records. I went with St. John, since the date of death matches the date of his death record.[1]
