If there is more than one city/town with the same name in the same state, then the county name in included in the category name to disambiguate.
In this case, it looks like there is only one Fanshawe, Oklahoma, but it's in two counties. So two categories were created for the same city, one in each county.
I'm not sure if that's how it should have been done in this case. I did a similar thing with Category:Bramshaw, Hampshire and Category:Bramshaw, Wiltshire, because there was no category name that would work for both. But in this case, we could just have a Category:Fanshawe, Oklahoma, which would be a subcategory of both the counties.
