Have an unusual naming situation (my own)

Have an unusual name situation (mine). I was born Mimi Wynne Finn, but my mother went back and legally changed my name on my birth certificate to her maiden name of Thompson. I grew up being involved in the Finn family and want to be a part of that heritage and have my name on the Finn Surname Index. I have also been married twice, first Overton and then Baker. How in the world do I put my own name in correctly here? And I may have already screwed it up starting with my original birth name.
