How do I change the default middle initial (post marrying)?

Question

After marrying, my grandmother took her maiden name as a middle name.  How do I change that on her profile?  

This is a common practice in my family, and there seems to be no place for entering a full name after marrying.

Last name at birth is an option.  Current last name is an option.  I am looking for a "middle name at birth" and "current middle name" as options.
