Help translating German military service record

Can someone provide help translating a German military service record from WWI?  It's on Ancestry at:

https://www.ancestry.com/discoveryui-content/view/1875724:1631?ssrc=pt&tid=168292664&pid=182461470822

The record of interest is for Leo Gutmann, the third (bottom) one on the page.  Ideally, I'd like the source identified and a translation of the record, including column headings.

Thanks.
