If I create a profile (or edit an existing one) and the LNAB is the Current last name, does the Current last name need to be filled?

It might fill it in automatically.  But you can't leave it blank.
R J is right; you cannot leave the field blank. It will automatically be filled with the LNAB. If you delete that and leave it blank, it will automatically fill the Current Last Name field with "Anonymous."

You have the birth name, married name and other. If male then there should not be any need for another name. If a female if known maiden name should be added, or married name added.
