Do I enter my name as my maiden name or my married name

You would put your maiden name in the "Last Name at Birth" (LNAB) space, and in "Current Last Name" put your married name.  That means that when you come to enter your parents etc.  the names tally.

Yeah, a question I can answer.  Your birth name goes under the LNAB--Last Name At Birth.  Further down the list is a place for Current name.  That is your married name if you use one different than birth name.  If you have been under any other name there is also a category for Other names.  It will come up as A.K.A.  This might be if your name is spelled other ways such as Smith and Smyth.  I hope this helps.
