When/how to use the middle name?

I know that in some countries (for example Spain), there are no middle names, but composite given names are common. Should this be done for long German names, or should some of the name go in the middle name field? For an American, I would assume that anything after the first given name should go in the middle name field? Are there any guidelines for separating compound first names from middle names? If someone changed/added their middle name during their lifetime, should you put the middle name at birth, or current middle name?

I would take a look at this thread, which talks about placement of the middle names and second first names such as you described.

Also, there is only one field for middle name, none for middle name at birth or current. Maybe you could specify that the middle name was changed/ added at a certain point in the biography.
