Names are determined for each individual person, not for an entire family line. (As you have seen, spellings can vary from one generation to the next, and often one person's name was spelled differently in different records.)

The Last Name at Birth (LNAB) generally should be the name found in a person's birth or baptism record (if any exists) or other record that indicates the name in use around the time of birth. The Current Last Name is the name they were using around the time of death (for example, sometimes this is the name on the gravestone). Other variant spellings or alternative names should be recorded in "Other Last Names" -- separated by commas.
