Where did Permelia Myers Wilkins die in 1914?

Wondering where Permelia died and why I cannot find any additional evidence...

I have documentation from Greenlawn Cemetery in Columbus, Ohio indicating she died in Gassaway, West Virginia in Feb 1914.  Her son, Clarence and his family, lived there for the 1910 census and was still living there in 1914.  I cannot locate Permelia in the 1910 census (either in Columbus or Gassaway).  I cannot find a death certificate in 1914 (either in Columbus or Gassaway).

Another option is that she was living with her grandson, Earl Wilkins and his family, in Charleston, West Virginia.  I know for a fact that they were skipped in the 1910 census (enumerator goes to the block before and never goes further).

Anyone aware of anything unique about Braxton County that would result in no death certificate for Permelia?  Or if there was a old age home where Permelia might lived in 1910?  Permelia died of dementia.
