Double listing on challenge tracker record

Question

I just tried to enter a death record from the profile to challenge tracker.
After I clicked add action I flipped back to the profile but apparently a
delay prevented the application.  I went back and checked and the add was not there so I pressed add again.  Then two identical actions came up in
recorded actions.  Is there a way to remove one or will Ales' machinery
eventually delete one of them?
