Is it correct to say "Utah Territory, United States' before Utah became a state in 1896.

I believe Utah Territory or Territory of Utah is correct.

I usually see it just "Utah Territory."  I think a good idea would be to provide an inline link to an article about the Utah Territory, possibly to Wikipedia or somewhere, so than anyone who wanted further information could get it easily.

Ether works. It is often left out because it was understood that Utah was a territory of the United States.

But, if United States is included in the birth or death location, it generates a 603 error - USA too early in time period.
