The great-grandfather of Majel Barrett Roddenberry was born in Bohemia as Joseph Hendrech. He changed his name legally to Hendrick..

So, last name at birth Hendrech. But , since his name was changed legally , would Hendrick go under "current last name" instead of "other last names" ?

Eddie, any idea where in Bohemia? Would like to look at the original as Hendrech doesn't strike me as particularly Czech, Hendrich would be much more common.

U. S. Census records for the family just say either Bohemia or Austria. The "Hendrick"  and "Hudec" (Majel's other line - also Bohemian -) all came to Cleveland Ohio circa 1880. 

This is my Majel free space page with research notes if you would like to study it.  I only started digging last night so there's not very much documented yet. Thanks for any help.

At the 50th anniversary Star Trek convention, they had a special memoriam booklet with Majel's ancestors and most of the names in the notes came from that . The other names, I called the Cleveland State University Library .
Most Czechs emigrated through Hamburg or Bremen. It's a reach, but checking the Hamburg passenger lists on Ancestry might help if they went that way. Those passenger lists often have a "last lived" town listed. Otherwise it would be basically impossible to find their hometown unless other documents show up.

I would say it was the current Last name, it was his legal name at the time of his death.

Thank you , Dale. That's my thinking. All his docs in Cleveland use Hendrick including his tax records .
