1776 Sticker Question [closed]

Question

Col John Allen served in the American Revolution from 16 Sep 1777 to 1 May 1781 serving Machias, Maine.  The 1776 sticker on the page shows a space for these details but I'm not sure how to add them.

Can anyone familiar with these stickers help?

To enter the dates add the lines:

| startdate = mmm dd, yyyy
| enddate= mmm dd, yyyy

after the 1776 and before the |unit= lines .
