My surname changed when I was a baby

When I joined wikitree they asked for my name at birth. My surname ( and my parents )was changed by deed poll when I was a baby. Now what do I do?

Is Cohen your original surname or the changed surname?

Personally, I would use the original birth surname (Last Name at Birth), especially if I had the original birth certificate/record.

I would view the deed poll as a simple name change, with the new surname your Current Last Name (or Other Last Name, if your surname has been changed after the deed poll).

WikiTree has exceptions for LNAB for infants adopted shortly after birth and for presumed spelling errors, but I don't think those exceptions would apply in your situation.
