Translation of czech baptism record for Anna Szczurek

Dear community

could anybody help me to translate the baptism record of Anna Szczurek? (see photo in her profile). She is my husbands great-great-grandmother

I hope I got the names of the parents right, but I am not sure whether I missed something else in the text.

Any help to find her parents / relatives would also be appreciated as my czech knowledge is limited to a few words.

Looks mostly correct to me -- her father's name in the record you linked seems to be "Blazeje", but maybe "Blasius" is the Latin form of it.

See below my translation of full birth record:

Father:
Ščurek Blažej, catholic worker in Jaroměř, legitimate son of Antonín Ščurek cottager in Bertovice nr.61 and his wife Marie, daughter of Jan Bierna from Norek nr. 54, district Myslenice in Halič.

Mother:
Kristýna, catholic, legitimate daughter of František Horáček, neighbour and carpenter in Jičín nr.93 (suburb/part Nové Město) and his wife Lidmila family Štolba from Kartouzy nr.5 district Jičín.

Godparents:
Anna Vítová daughter of Ferdinand Vít, hřebenář(= man producing combs) in Jaroměř nr.149
and
Marie Jarošová wife of Hynek Jaroš, worker in Jaroměř nr.139.
