On her deathbed, my g-grandmother told her adult children the family name. For their whole lives, the kids had been told by my g-grandmother that their parents fled Poland because of a scandal. On her deathbed, she requested to be buried under the family name, which she told the kids. The kids, who spoke Polish, but didn't write Polish, put on her death certificate the name as they heard it. Until this revelation the family name was Fisher, so when my great-grandmother came out with Grazdofski, they didn't have a clue how to spell it and g-grandma didn't write.
