How to format former middle names?

I'm going to use myself as an example. I was named Søndergaard as my middle name when I was baptized, but I changed my middle name to my wifes, when we got married, but I'm unsure how to make it clear that my name is no longer Søndergaard but it was my baptized name?

You should use the name given at birth and explain the name change in the biography.
