How to Cite a Family Group Sheet?

I have a Family Group sheet that was handed down to me, along with many others. I know some were done by a pro. genealogist, some by my aunt in conjunction with the Detroit Genealogical Society, and some -well I'm not sure! What I'm looking at is an LDS document, on the bottom it says, (Joseph Fielding Smith, Church Recorder, Printed in U.S.) This sheet is for Frost-3588 and his issue. The profile hasn't been modified since 2017 and while there is a bio, there is only sources. However, the information is EXACTLY the same as what I have on my group sheet, leading me to believe it was entered by someone else with a copy of what I have.

The right side of this form has the information for the LDS Church claiming baptisms ranging from 1909-1933 and recorded in the Los Angeles temple by the 10th great granddaughter, Ethel King Millar, 9 AUg 1960. The document has one citation: "Old Kittery and Her Families" by Everett S.Stackpole pg. 413-414.

Can I cite this document?  If so, how?

preferably that is not the only source citation you have, a family group sheet is not very different from a family tree.  Cite the original sources as much as possible to back it up.
Of course. The profile is project protected, so I put a few of my other sources on the bottom. They may to choose to use them or delete them. I have more, but that particular project relies on one particular source above all others, so it is their choice.
don't know which project is involved, don't particularly care, if your source is valid it is valid.  Some people prefer certain sources, but excluding any others is counterproductive.

A proper citation would be something like:

Author Name, John Smith - Jane Doe Family Group Sheet, Smith Family Group Sheets and Charts, Supplied by Your Name, [address if appropriate], State, Country, date-from-sheet. description of what the sheet covers and whether there is documentation.

This is based on what Evidence Explained has in sections 3.28. Supplied by is who provided you with the material. It can also be stated as privately held with your name (if you created it) or just privately held if the owner has not given permission to be identified.
