How do I include someone who has two names, a birth name and adopted name?

In the name fields, you'll want to enter her surnames as follows:

Last Name at Birth:  Birth Name

Current Last Name: Current Last Name that your mother uses

Other Last Names:  Adopted name and any other surnames that she has used, separated by commas, if different from her Current Last Name.
