This photo appears to have been taken at the foot of the Alps Mountains, which means it was taken after the end of the hostilities on May 2, 1945.

The markings on the Jeep indicate it belongs to : 349 Infantry Regiment, 3rd Battalion, jeep number 4. The insignia on his helmet is not a medic red cross but the Blue 4-leaf clover (with wide White edging) insignia of the 88th Infantry Division. The 88 Division remained as occupation force in Italy and some stayed longer and became the TRUST Forces (short for Trieste US Troops), which adopted the 4-leaf clover insignia and added a red shield in the center.

A tough medic on a Willys jeep in the service of his country in war torn Italy gets my vote. I like how the photo bears his signature, too.
