Notes

There are quite a few documents from The Mental Health Institute of Independence, Iowa, obtained by a relative in 2009, that show the admission and death of Lydia Carr Kilbourn. The forms also contain false statements, and it is difficult to know how the information was obtained, and what to trust on these documents. This medical information was obtained through a third party, and it is not currently possible to provide the source without further explicit permission.

Lydia Celestia Carr's insanity commitment records - July 6, 1891

QUESTIONS on admission form:
  1. What is the patient's name and age? Married or single? If children, how many? Age of youngest child? Lydia C. Kilbourne age 62 years. Widow. Mother of twelve children. Four living. Youngest child's age 20 years.
  2. (A) Where was the patient born? (B) Where was the patient's father born? (C) Where was the patient's mother born? Patient born in New York. Father born in England. Mother born in England.
  3. Where is his or her place of residence, (legal settlement.)? Clarion, Wright Co., Iowa
  4. What has been the patient's occupation? Housewife
  5. Is this the first attack? If not, when did the others occur and what were their duration? The first manifestation of Insanity was at the time of birth of youngest child and has been noticeable frequently since that time.

(Note-- many of the answers above are extremely unlikely, and some are certainly false. How much can any of this information be trusted? She is only known to have had six children, although there are enough gaps between children that it's possible that several died. She is not widowed, but divorced. Her mother and father were both born in Rhode Island, USA, not in England. One of her known children (Mary) apparently died in late 1870, but the other five can be found alive on census records after Lydia's death)

Unidentified, (apparently medical) form that gives Lydia's death (and other) information:

Name: Kilbourn, Lydia C.
County: Wright
Number: 5105
Date of Admission: October 7, 1891
Number of Admission: First
Age: 62 years
Civil Condition: Widow

Skipped several pieces of information on the form, which ends with:

Date of Discharge: December 21, 1898
How Discharged: Died 5 a.m.

Queries about the surname Kilbourn, requesting information about Hiram Kilbourn and Lydia C. Carr, stating they were married in 1833 (that's actually when Lydia was born, but the poster probably got the facts confused and mixed). It states they were divorced in 1868, and that she died in Independence, Iowa (no year given). Another response was from a descendant of Lydia's son Francis, and gives a few more clues, including they had heard that Lydia died in Independence, Buchanan Co., Iowa, but didn't have a date or source of the info.
