King George III in his reign began the tradition of gifting miniatures of himself to be worn suspended from a ribbon. The tradition was formalized by his son, King George IV. Many courtiers in the court of George III, both men and women, had participated but George IV limited the formal order to be only a gift to women. The tradition was continued by every sovereign of the United Kingdom up to the modern day, except for William IV and Edward VIII.

While typically limited to members of the royal family, the first documented recipient was Princess Dorothea Lieven, the wife of Prince Christoph Heinrich von Lieven (the Russian Ambassador to London)
