Dr. abbreviated my dad's first name on his birth certificate. Dad went by Charles his entire life, but had to submit a letter to the Commandant, US Merchant Marine Academy stating this fact and that he went by Charles in order to receive his diploma when he graduated.
I have copy of his birth certificate and the letter sent to the Commandant prior to his graduation.
