Frederick Cook was born in the Colony of New South Wales (1788-1900)
Frederick William Cook was born on 21 October 1883, at Sydney, New South Wales. He was the son of Joseph Blain Cook (1851-1920) and Eliza Franklin White (1862-1919).

Frederick married Lily Regina Reith (1898-1973) in April 1920, at Hampstead, Middlesex, England.[1] They had four sons:
- Charles Blain Cook (1921-2003)
- Clive Leonard Cook (1922-2011)
- Roy Francis Cook (1923-1971)
- David Geoffrey Cook (1927-2000)
He passed away on 31 July 1973, at North Ryde, New South Wales.[2]
