Biography

Floyd Harrison Cook was born on November 24, 1881, in Wheeler, New York[1][2].

In 1900, Floyd (18) lived as a boarder, at 25 Chapel Street, in Seneca Falls, New York and worked as a telegraph operator[3].

On October 25, 1909, Floyd was a clergyman and married Cora Belle Baker[4].

In 1910, Floyd (28) lived on Linden Avenue, in Middletown, New York, where he worked as a medical doctor[5].

In 1911 - 1912 Floyd and Cora had a daughter Eulala Cook and a son Floyd Harrison Cook [6].

On June 1, 1915, Floyd (34) lived in Middletown, New York with his wife, Cora, son, and daughter.

Floyd enrolled on April 6, 1917 and served in the World War I Military Service, 1917-1919, "to duty under instruction Naval Medical School, Washington, D.C."[7][8].

In 1920, Floyd (38) lived at 32 Leno Place with Cora (38), Eulala (8) and son Floyd (7); Floyd worked as a physician[9].

In 1923 Floyd and Cora lived in Middletown, New York.

In 1930, Floyd (48) lived in Akron, Ohio, as a "Guest" with 4 lodger's, 2 other guest's and 1 roomer; Floyd continued to practice as a physician at his own practice[10].

Floyd, at the age of 55, took a trip to the West Indies on December 19, 1936 and arrived in New York, on January 3, 1937[11].

On December 3, 1938, in Akron, Summit County, Ohio, Floyd married Mary Helen Burns[12].

In 1940, Floyd (50 - as it's written in the census) lived at 727 Dorchester Road in Akron with his wife Mary (29); Floyd worked as an Oculist in his own practice[13].

In 1942 Floyd (58) was living in Summit, Ohio, when he registered for the World War II draft [14].

Military Service

Dr. Floyd H. Cook served in World War I, as a Lieutenant in the Third Naval District. He was called to duty on April 17, 1918 under instruction of The Naval Medical School in Washington, D.C.[15].
