There is a conflict as to the death of Samuel Frazier. The approved death packet cover has the death date for Samuel Frazier, Roll# 1812 as 2nd of February 1904.[2] The "Affidavit of Relative" attested to by Simeon Morris and Davis Homer both Subscribed and Sworn before O. T. Gregory, Notary Public, on 8 February 1904 that Samuel W, Frazier, Roll# 1812 died on 5th of February 1904.[3]
To add to the confusion, on 24th of October 1907, Samuel Frazier, Roll# 1812 provided testimony in the matter of the births and deaths of Martha Frazier and Eliza Frazier.[4][5][6]
The sources listed from the “Oklahoma Applications for Allotment, Five Civilized Tribes, 1899-1907” show the Oklahoma and Indian Territory, Land Allotment Jackets for Five Civilized Tribes, application for allotment affidavit, Choctaw Citizenship Certificate.[7][8][9]
