BIOGRAPHY
Feuyt was born in Warmond in 1633 as the son of Jan Jansz and Jannetgen Feuyten.
His father and older siblings carried the surname Bout at their baptisms in prior years.
Based on available records, Feuyt was the first member of this family documented as having taken the van Leeuwen surname in one form or another.
In 1657, he was married (as Feuyt Janssen van Leuwen) to Jannetge Pieters van Alkemade;
in Jan of 1660 to Grietje Ewouts Blom;
and in the 1680s, he had two children with Aaltje Cornelis.
Feuyt and Jannetge Pieters had difficulty getting approval for their marriage, until they went to her hometown.
They had two children, Grietje and Johannes.
Feuyt marries Blom in Jan 1661, less than two months after the birth of Johannes, the son born in Nov 1660 to his prior wife, Jannetge Pieters.
The request for this 2 Jan 1661 wedding was first made 12 Dec 1660.
Pieters must have died shortly after childbirth.
Feuyt and Blom's first child (Cornelis) was born less than eight months after their wedding.
Johannes (born 1660) died as a young child, but likely not until after Cornelis was born in 1661.
As a father, Feuyt named his first and third born sons Johannes, presumably after the formal name of his father (and/or grandfather).
Otherwise, the decision to give the name Johannes to the son born to them in 1662 (signaling the death of Johannes born in 1660) would likely have been made in 1661 to their son (baptised as Cornelis) rather than to the second.
Someone named Foyt van Sijp and/or Foyt van Leeuwen managed a molen in Warmond during several periods from 1662 to 1694. See below.
Feuyt passed away about 1694 (based entirely on when his presumed association with management of a windmill ceased.)
SURNAME CHANGES
There is no known prior use of the last name van Leeuwen, or its variants, by anyone in the Bout family, prior to 1657.
Thereafter, his surname was recorded as van Leuwen, and then only later as van Leeuwen.

1657 - Married as Feuyt Janssen van Leuwen, per both the WWW/OA file records and in the original document.
1660 - Married as Foijt Janssen van Leuwen; sister marries as Magtelt Jans van Leuwen
1661- 1665 - No surnames recorded for father or children at baptisms.

(per WWW; document itself raises doubts as to the recorded surname.)

(All the surviving children of Jan Gerritsz and Annetgen Engelsdr. -- meaning by this time, Gerrit alone (Feuyt's uncle) -- and their grandchildren and/or great grandschildren including Feuyt .... Daughter Geertgen married twice -- to Cornelis Cornelisz Boon, and then to Cornelis Maertensz Bols; Pieter Benjaminsz Maarlevelt was a son of Machtelt Jansdr. Jan Jansen most likely is a grandson.)
WARMOND MOLENAARS
A Jan Jansz and a Foyt Jansz alternate management of a Warmond mill (with others) using first van Sijp and later van Leeuwen as surnames, over the period between 1651 and 1694.
It appears possible that Jan Jansz van Sijp and Jan Jansz van Leeuwen are the same individual;
similarly for Foyt Jansz van Sjip and Foyt Jansz van Leeuwen.
However, Jan Jansz van Sijp could be the brother of Jan Jansz Bout and an uncle of Foyt's.
Jan Jansz van Leeuwen could also be Jan Jansz de Jonge although his occupation is recorded as bondsman.

Jan Jansz van Leeuwen 1661
Foyt Jansz van Sijp 1662
Foyt Jansz van Leeuwen 1664
