Theodore was born 22 Jun 1902 at Conway, Pontotoc, Chickasaw Nation, Indian Territory.[2] He is the son of George Perry and Annie Walker.
The Dawes Commission transferred all the individuals listed on the Choctaw Enrollment card 25[3]. The Commission then cancelled this card and moved all the individuals to the Chickasaw enrollment card 1585[4] on 11 October 1902.
