Phyllis Jane Sisk was born 21 February 1943 in Wellington, Collingsworth, Texas. She was the daughter of Robert L. Sisk and Helen Sue Singley,[1]
She passed away 3 January 2016 and was buried at Valley View Memorial Park, West Valley City, Salt Lake County, Utah.[2]
