  • I certify, on honor, that Gilbert Willett, a Priv. of Captain Danl Coates Company (F) of the 6th Regiment of USC Cav, born in Union Co., State of Kentucky, aged 44 years; 5 feet 3 inches high; Blk complexion, Blk eyes, Blk hair, and by occupation a Farmer, who joined for service and was enrolled as a recruit (see Note 9) on the 24th day of Novr, 1864, at Henderson Ky by Capt Johnson, for the period of three years, and mustered into the service of the United States on the 12th day of December, 1864, at Owensboro Ky, by Capt Grissom; and having served HONESTLY and FAITHFULLY with his Company to the present date, is now entitled to a DISCHARGE by reason of death. Died in Hosp at Duvalls Bluff Ark Jany 19 ’66 of Pneumonia.
  • The said Gilbert Willett was last paid by Paymaster Maf. Van Valkenberg, to include the 31st of October , 1865, and has pay due him from that time to the present date; he is entitled to pay and subsistence for TRAVELING to place of enrollment, and whatever other allowances are authorized to volunteer soldiers, drafter men, or militia, so discharged. He has received from the United States CLOTHING amounting to $53-00/100 dollars, since the ___ day of Enlistment when his clothing account was last settled. He has received from the United States $100-00/100 dollars advanced BOUNTY.
  • There is to be stopped from him, on account of the State of _____, or other authorities, for CLOTHING, &c., received on entering service, _____________ dollars; and for other stoppages, viz: ____________ dollars. He has been furnished with TRANSPORTATION in kind from the place of his discharge to ______; and he has been SUBSISTED for TRAVELING to his place of enrollment, up to the ________, 186_.
  • He is indebted to __________, SUTLER, _________ dollars.
  • He is indebted to __________, LAUNDRESS, ________ dollars.
