Ohio has 32 counties in Appalachia: Adams, Ashtabula, Athens, Belmont, Brown, Carroll, Clermont, Columbiana, Coshocton, Gallia, Guernsey, Harrison, Highland, Hocking, Holmes, Jackson, Jefferson, Lawrence, Mahoning, Meigs, Monroe, Morgan, Muskingum, Noble, Perry, Pike, Ross, Scioto, Trumbull, Tuscarawas, Vinton, and Washington,[2] which are in the Northern and North Central Appalachia Regions.

- 1846: Mahoning County was created from Columbiana and Trumbell Counties.[4]
- 1846: Morgan gained from Washington.[4]
- 1848: Tuscarawas gained from Stark.[4]
- 1850: Vinton was created from Gallia, Athens, Ross, Hocking.[4]
- 1851: Noble County was created from Guernsey, Monroe, Washington, and Morgan[4] (all but Guernsey County are in today's North Central Appalachia Region).
- 1874: Highland gained from Brown.[4]
