Misao was born in Osaka, Japan, on March 5, 1898. She was the daughter of a draper.
Misao married Yukio Okawa in 1919 and had three children, of which two survived her. Her husband died on June 20, 1931.
After the death of Koto Okubo, Misao has been the oldest living woman in Japan and throughout the world. Upon Jiroemon Kimura's death, Misao was also the oldest living person in Japan and the world.
Misao Okawa died from heart failure in Osaka, Japan, at 6:58 AM on April 1, 2015, at the age of 117.
