Ballard Jacob Warner was born 30 December 1894 at Tobaccoville, North Carolina, United States of America. He is the son of Sidney Lewis Warner and Sarah Jane Masencup.

Historical documents record Ballard as Ballard J Warner and Ballard Jacob Warner. Census records record that he was born December 1894 in in North Carolina; at 15 he was attending school and was able to read and write; he had an eighth grade education; at 25 he was working as a house carpenter; he was 29 when he married Jessie; at 35 he was a cabinet maker in a furniture factory; at 45 he was a Mill Work Superintendant in Building Materials Industry. Historical documents record that he was a retired Shop Foreman of a Lumber Company. He was described as being tall with a medium build, brown eyes and dark brown hair.

Ballard is recorded in the 1900 United States Federal Census, enumerated 16 June, as Ballard J. Warner residing in the household of his parents Sidney L. and Sallie J. Warner in Old Richmond Township, Forsyth County, North Carolina. His birthdate is recorded as December 1894, he is five years old. Also residing in the household is his brother one year old Crescent E. His paternal grandparents, Jacob E. and Adline Warner, are residing one residence down.

Warner on a farm on Seward Road in Old Richmond Township, Forsyth County, North Carolina. He is 25 years old, single and acarpenter in the housing industry. Also residing in the household are his brothers 20 year old Cresent E., 18 year old Grady F., 16 year old Arnold and 13 year old Newell C. and sisters nine year old Ethel V. and six year old Sadie L.

He married Jessie Frances Shore 07 June 1924 at the home of C.R. Law South Fork Township, Forsyth County, North Carolina. Jessie was born 26 August 1905 in Oklahoma; she is the daughter of John Henry Shore and Rosalie Steelman. Ballard was 29 years old and Jessie was 18 years old as recorded on their marriage certificate. W.L. Dwiggings and Mrs. W.L. Dwiggins of Mocksville, North Carolina and A.K. Waskelis of Winston-Salem, North Carolina were witnesses to their marriage by [C.R.] Law, a Minister of the Church of the Brethren. Ballard was a resident of Tobaccoville, North Carolina and Jessie was a resident of Cana, North Carolina. They had one known child.

Ballard is recorded in the 1930 United States Federal Census, enumerated 15 April, as Ballard J. Warner head of household renting a house for $7.00 per month in Old Richmond Township, Forsyth County, North Carolina. He is 35 years old and is a cabinet maker in a furniture factory. Also residing in the household is his wife Jessie F. Warner.

In 1935 Ballard and Jessie were living on Clemmonsville Winston Salem Road in Clemmonsville Township, Forsyth County, North Carolina.

Ballard is recorded in the 1940 United States Federal Census, enumerated 18 April, as Ballard J. Warner head of household residing on Clemmonsville Winston Salem Road in Clemmonsville Township, Forsyth County, North Carolina; he owns his house valued at $800.00. He is 45 years old, has completed the eighth grade and is a Superintendant, Mill Work in a Building Material Company. Also residing in the household is his wife Jessie F. Warner.

His father died 2 February 1947 at Winston-Salem, Forsyth County, North Carolina and his mother died 5 May 1956 at Winston Township, Winston-Salem, Forsyth County, North Carolina.

Ballard was 66 years old when he died 2 December 1961 at Winston-Salem, Forsyth County, North Carolina. He is buried at Enon Baptist Church Cemetery in East Bend, Yadkin County, North Carolina.
