How do I write German in a French questionnaire

Want to do a bit of research into this site but unsure of how to put my information in.  Do understand a limit3ed amout of French but.........

 

Nom de famille
Localité ou paroisse d'origine
Département, état ou pays
Lieu du mariage

 

 

Realize the family name which is Albrecht or Albright goes into the first section plus the 2rd spot if for  Parish (don't have one) the 3rd spot is for Country which is Allemand (??) & the last one is perhaps a marriage date?  Can anyoe help me here?

Locality (place) or Parish of origin.

Germany in French would be Allemagne, Allemand are the people living in Allemagne

The last one is where the marriage took place not the date.
