"He had drawn a prop from the waste and was in the act of drawing a second one when a large stone fell from the roof and killed him instantly. The stone was 16 feet long, 3 feet wide, 11 inches thick, and would weigh about 45 cwt. A ringer and chain should have been used, and if it was necessary to loosen the props, temporary timber should have been set."
