Annie Elizabeth Fountain was born on 18 November 1858 in Newton Township, Muskingum County, Ohio, USA. She was recorded there as 2 years old, living in the household of her parents: Martin Fountain, age 26, born in Ohio, and Annie [Sarah Ann Attwell] Fountain, age 25, born in Ohio. She was their only living child as recorded on the 1 June 1860 US Census.^[1][2]

Her maiden name is given as Fountain on the Licking, Muskingum, Ohio, USA, birth certificate of her daughter Florence.
