The application was made by her self, Florence Barkhouse, who was living in Hantsport, N.S., on 30 March 1963. The affidavit was signed by Eric W. Smith in Hantsport, Hants County, Nova Scotia on 30 March 1963.

All except Fred were born in N.S. They were living in Avonport, in the Township of Horton, in the District of Kings, in Nova Scotia in 1901.
