Theodore was born in 1921 in Ohio, United States. [1] He was the son of August and Frances Otcasek.[2]
His siblings were: William, Oscar, Ladislaw (Ladd), Albert, and August Jr. [2]
He died in 2003 [1]and is interred at Fort Rosecrans National Cemetery.[3]
