Forman died 1882 May 13th possibly in Ocean County [2]. His memorial is in the same plot with Eliza Ann VanMarter, Charles, Maria, Charlie Thompson and a stone marked SSM in the Pleasant Grove Cemetery. His memorial is inscribed:
