State of Pennsylvania, Chester County SS.
On this Second day of July A.D. 1868 before me a Justice of the Peace in and for the county and State above named personally appeared Rebecca Clark who being affirmed says that she is an applicant No 75141 for Increase of widow's Army Pension. That her son Samuel Clark was born on the 22nd day of April A.D. 1853.
That the statement in her application was intended to be above statement and no other.
