I came out of the army. Got it at Honey Hill Nov 30 /64 & wound of right hand & left hip and left wrist at Honey Hill Nov 30 /64 Right Hernia near Savannah Geo. Spring 65 by a fall & [TBD] ever since war & have had rheumatism past 6 years & eyes are very bad since discharge.
That said disabilities are not due to his vicious habits, and are to the best of his knowledge and belief permanent. That he has applied for pension under application No. 1,028,602. That he is a pensioner under Certificate No [blank]
That he makes this declaration for the purpose of being placed on the pension roll of the United States under the provisions of the Act of June 27, 1890. He hereby appoints CHARLES K. GIBSON, of Grand Rapids, Mich., his true and lawful attorney, to prosecute his

On this 29 day of June A.D. 1893 personally appeared before me, a Notary Public within and for the County and State aforesaid, James H. Jackson aged 65 years, a resident of the town of Grand Rapids, County of Kent, State of Mich, who, being duly sworn according to law, declares that he is the identical James H. Jackson who was Enrolled on the [blank] day of [blank], 1863, in Co[mpany] "D" 54th Mass[achusetts] Col[ored] Inf[antry] in the war of the rebellion, and served at least ninety days, and was Honorably Discharged at Beaufort Is. S[outh] C[arolina], on the Fall, 1865. That he is unable to earn a support by [TBD] of Total deafness right ear. Have been deaf ever since
