Applicant was wounded at the charge on Fort Wagner which was on the same day that Col Shaw of said regiment was killed. After receiving said wound applicant never afterwards did any active service. He was sent to Camp Hospital after being wounded where his wound was dressed by regimental surgeon, next morning he was put in an ambulance and sent to Beaufort Hospital No. 3 where he re[TBD]ed to the first of March following being then unable to do duty

he had a sick leave of 30 days to go home at Sidney Shelby Co, Ohio, before the expiration of 30 days I went t to the examining surgeon at Columbus Ohio to get my furlough extended.

After an examination the Dr. told me to go to my regiment and I would be discharged as I would be unable
