Frank Leslie Andrews was born on June 24, 1858 in Kendallville, Wayne Township, Noble, Indiana, United States, son of William Andrews (1818–1895) and Laura Ann Maynard (1823–1910).
Frank died on February 22, 1895 in Posey Township, Rush, Indiana, aged 36. He was buried in Arlington East Hill Cemetery, Arlington, Rush County, Indiana.
