Forman was born about 1812 to Ezekiel Applegate and his wife, Hannah Perrine [1].

Forman was married 1833 May 6th in Monmouth County to Elizabeth Clayton [2]. Their children are Elizabeth (Mount) [3], Baird W [4], George, John, Thomas F [5], Hannah (Hunt) [6], Asa [7], Matilda (Cox) [8], Jacob [9], and Jane.
